How to use cover page templates
A fax cover page tells the receiving office who the fax is for, who sent it and how many pages to expect. In a busy clinic or law office where one fax line serves dozens of people, that first page decides whether your document reaches the right desk. Typing the same details every time is tedious and leads to mistakes. This faxapp.ca guide explains what a good cover page contains and how to set up reusable templates.
Get Started NowWhat to put on a fax cover page
Keep sensitive details off the cover page itself. A name and a reference are usually enough; diagnoses, account numbers and similar information belong in the pages behind it.
- Your organisation's name and logo.
- To: the recipient's name, department and fax number.
- From: your name, phone number and fax number, so they can reach you if pages are missing.
- Date and the total number of pages, including the cover page.
- Subject or reference, such as a file number, claim number or patient reference used by the recipient.
- A short message saying what the fax is and any action needed.
- A confidentiality notice asking anyone who receives it in error to notify you and destroy the copy.
Create a cover page template
FaxToFax.com, the service faxapp.ca recommends, lets you add your logo to cover pages and save reusable templates. The exact screens may change as the service develops, but the process follows this pattern:
- Sign in to your account in any modern browser.
- Open the cover page settings and upload your logo.
- Fill in the details that never change: organisation name, phone number, fax number and confidentiality notice.
- Leave fields like recipient, subject and message to fill in for each fax.
- Name and save the template so it is easy to find, for example "Referrals" or "General correspondence".
Use different templates for different jobs
One generic template is better than none, but a few specific ones save more time. Ideas for Canadian organisations:
- A clinic referral template with a stronger confidentiality notice and space for the receiving clinic's reference.
- A legal correspondence template noting that the contents may be privileged.
- An accounts template for invoices and remittances.
- A bilingual English and French template if you correspond with offices in Quebec or New Brunswick.
Send a fax with your template
- Start a new fax and attach your PDF, PNG or JPG, or write the letter in the editor on your letterhead.
- Choose the cover page template that fits.
- Fill in the recipient, subject, page count and message.
- Enter the fax number, or up to 50 numbers for the same document, and confirm each one.
- Send now, or schedule up to 90 days ahead. You can cancel a scheduled fax before it sends.
Keep templates up to date
Review your templates whenever a phone number, address or logo changes, and remove ones nobody uses. On a team account, agree who is allowed to edit them so staff are not surprised by changes. Out-of-date contact details on a cover page are a common reason replies go astray.
It is also worth sending a test fax with each new template to a colleague or a second number before using it with clients. Check that the logo is readable in black and white, that nothing important sits too close to the page edge and that the confidentiality notice is not so small it disappears.
How to Use Fax Cover Page Templates: questions
Is a fax cover page required?
Usually not by law, but many offices expect one, and it helps the right person get your fax. Some recipients specify their own cover sheet, so follow their instructions when they do.
Should the page count include the cover page?
Most offices count the cover page in the total, but state it clearly, for example "5 pages including cover", to avoid confusion.
Can I put my logo on a fax cover page?
Yes. FaxToFax lets you add your logo to cover pages. Simple, high-contrast logos look best, since faxes print in black and white.
Does the cover page use a plan page?
A cover page is a page of the fax, so plan on it counting toward your page total like any other page.
